Description

We provide a range of services within the Port Stephens area:

Food Services - Meals on Wheels, Breakfast Packs, Hospital Discharge Packs, Frozen Meals ( Frozen Meals available to the general public)

Social Inclusion Services - Home Visiting, Group Outings, Social Support One to One, Assisted Transport to appointments,Shopping Assistance, Library Run and Light Gardening. 

Aboriginal Services - Social Inclusion Services, Social Outings, Low Level Maintenance, Social Support One to One, Assisted Transport to appointments, Shopping Assistance, Fortnightly social outings for younger people with a disability - Aboriginal Peer Support Program and Port School Options. 

Linen Exchange Service - Provision and exchange of clean for soiled linen twice a week

Activity Centre - Centre Cased Activities for aged people Mon-Fri, Sundowners Bus Tours (Demetia Clients)

Asssessment - The assessment is our way of gaining useful information, and to determine the need for service. Priority is given to those who are at risk of premature admission to a nursing home
